要旨

【緒言】清代の『時病論』は季節ごとに発病しやすい病気の弁証論治を述べています。一般的に外感病の書に分類され、気象医学書としての評価はされておらず、『時病論』伏気説の文献的検討もされていません。

【目的】(1)気象医学書として評価されている『黄帝内経(以下内経)』との比較を通して、『時病論』が気象医学書として評価できるか、(2)古医書における「伏気」説と『時病論』伏気説の特徴、について文献的に検討しました。

【方法】(1)『内経』気象医学の病因説に基づき、『時病論』の記載内容を抽出して一覧表を作成し、比較検討を行いました。
(2)伏気説の調査には中華医典を用い「伏気」をキーワードとして、『時病論』までの伏気説を調査しました。

【結果】(1)『時病論』の気象・気候病因説は『内経』を基礎に構成されている。その中でも六淫病因説を基本とし、他の病因説を加えて病因病機を記載していました。『時病論』は外感病だけでなく雑病に関しても気象医学の病因説を用いていました。
(2)『時病論』は六気すべてが伏気になるという伏気説を、初めて体系的に述べた書であることがしめされました。

【考察・結語】『時病論』は(1)『内経』と同じ気象・気候病因説を採用、雑病への気象・気候病因説の運用、季節ごとの構成、雨季に対応した弁証論治などの点から気象医学書であると評価できるとしました。(2)『内経』の病因説に六気すべてが伏気になるとした伏気説を加え、『内経』の気象医学を発展させて、『時病論』の伏気説は「治未病」の観点から臨床応用可能と考えられます。

Review of literature on "The Shi Bing Lun"――Evaluation as Meteorological medical text and about "Fu Qi"

Introduction:
In the present age, concern about the influence on the body due to weather change is increasing due to frequent occurrence of abnormal weather and climate change such as global warming.
Qing dynasty's "The Shi Bing Lun" states "syndrome differentiation and treatment" of disease that is susceptible to on a season-by - season basis.
However, it is generally classified as a form of exogenous acute febrile disease and has not been evaluated as a meteorological medical text. There has not been any literature review of Fu Qi Theory in "The Shi Bing Lun".

Objectives:
(1)Through comparison with "Huangdi Neijing" which is evaluated as a
meteorological medical text, examine through literature whether "The Shi Bing Lun" is a meteorological medical text, and
(2)Examine the characteristics of "Fu Qi" theory and "The Shi Bing Lun" in the old medical books.

Methods:
(1)Content of "The Shi Bing Lun" was extracted and a list was prepared and compared based on meteorological/climate etiology of the "Huangdi Neijing".
(2)Using the Encyclopedia of Traditional Chinese Medicine, "The Shi Bing Lun" was examined with the keyword "Fu Qi".

Results:
(1)Meteorological/climate etiology theory in "The Shi Bing Lun" was compiled based on the "Huangdi Neijing". Among them, based on "pathology based primarily on six excesses", etiologic diseases were described including other etiologies. "The Shi Bing Lun" used etiology of meteorological medicine not only for exogenous acute febrile disease but also for miscellaneous disease.
(2)"The Shi Bing Lun" clearly showed that it was the first systematically stated book for the Fu Qi theory that tells that all the six excesses would be Fu Qi

[Investigation and Conclusion]:
(1)"The Shi Bing Lun" can be considered as meteorological medical text because of the adoption of meteorological/climate etiology theory same as "Huangdi Neijing", practical use of "meteorological/climate etiology theory" for miscellaneous diseases, season wise composition, "syndrome differentiation and treatment" based on rainy season.
(2)The medical meteorology of "Huangdi Neijing" was developed by adding the "Fu Qi Theory", which states that all the six excesses (factors) would be "Fu Qi". It seems that "Fu Qi Theory" of "The Shi Bing Lun" can be clinically applied from the viewpoint of "Prevention first".
